This workshop will explore how Charting the LifeCourse Framework and Principles can be used to reframe the conversation about community support and working together to expand access and deepen engagement.
Session 2: Building Good Lives
April 9, 2026, Time: 9 – 11 a.m.
This workshop will explore how using the Charting the LifeCourse Framework and Principles during the person-centered planning process assist individuals and families set goals and objectives that will lead them to their good life.
Session 3: Arrangements that Support Self-Determination (Self-Directed Services)
May 14, 2026, Time: 9 – 11 a.m.
This session will explore the Principles of Self-Determination and the Charting LifeCourse Framework and Principles to assist individuals and families in setting up arrangements that support self-determination through the person-centered planning process.
Session 4: A Good Life for Children, Youth, and Families
June 11, 2026 Time: 9 – 11 a.m.
This session will explore the Family Driven Guided Policy through the Charting the LifeCourse Framework and Principles and discover how the framework can enhance planning with children, youth, and families.
Identify the Charting the LifeCourse Principles and values that are important as we begin to think differently what it means to support individuals with disabilities to have good lives.
Identify how Charting the LifeCourse Framework and Principles can be used to reframe the conversation about community support.
Explore and identify how the Principles of Self-Determination and Charting the LifeCourse Framework is used to assist individuals and families in setting up arrangements that support self-determination.
Discover how Charting the LifeCourse Framework can enhance planning with children, youth, and families.
Social Workers: This course qualifies for a maximum of 8 Continuing Education hours. The Community Mental Health Association is an approved provider with the Michigan Social Work Continuing Education Collaborative. Approved Provider Number: MICEC 060818. Qualifies as “face-to-face" (in-person) education.
Substance Use Disorder Professionals: CMHA is approved by Michigan Certification Board for Addiction Professionals (MCBAP). CMHA maintains the responsibility for the program and content. Substance Use Disorder professionals participating in this training may receive a maximum of 8 related contact hours. It is important that attendees keep a copy of the training program brochure containing the workshop description.
Jan Lampman, BA, QIDP, CAPP, has worked with individuals with Intellectual and developmental disabilities since 1985. Starting as a direct support professional in her last year at Saginaw Valley State University, Jan developed a passion for inclusion and community for all citizens. Upon graduation from SVSU, Jan continued her work with people with I/DD as a group home manager, opening five homes to allow individuals to leave a state institution. Jan then moved on to The Arc of Midland, where she worked for 27 years, developing new initiatives, and helping to create a culture in her community that supports, values, and includes people with I/DD. Jan was a founding member and faculty of the Michigan Academy for Self Determination. Jan was also a consultant for the Center for Self Determination. Jan is currently the owner of Community Drive, a consulting and training organization dedicated to promoting self-determination, person-centered planning, community, positive change and thriving.
Justin Tate, LMSW, is the Manager of the Family and Community Partnership section in the Office of the Advocate for Children, Youth and Families in the Bureau of Children’s Coordinated Health Policies and Support at MDHHS. Previously, he has worked as the Manager of Programs for Children with SED and as the Statewide Wraparound Coordinator during his seven years with MDHHS. Prior to working at MDHHS, Justin worked in the community mental health system, where he worked as a mental health therapist and wraparound facilitator for 10 years. Before joining the CMH system, Justin worked with homeless and runaway youth in the Lansing area. Justin graduated from Michigan State University with a bachelor’s degree in Family and Community Services and went on to earn his master’s degree in social work at Michigan State University in 2011.
Angela Martin, LMSWA, is the Senior Associate Director at the Michigan Developmental Disabilities Institute (MI-DDI) at Wayne State University in Detroit, Michigan. Ms. Martin has a master’s degree in social work from Wayne State University. She has experience in resource development, curriculum design, and training programs on self-determination, person-centered planning, family support and leadership/self-advocacy. She has been the Project Director/Principal Investigator for several statewide projects on self-determination, family support, and quality community-based services and support.
Beth Durkee, LMSW career is driven by a commitment to fostering independence and meaningful lives for people with disabilities. She specializes in person-centered planning and self-directed services, empowering individuals to lead self-determined lives and to access supported living and community inclusion. With a background including supported employment, robust program development, and creating innovative solutions, she also offers comprehensive family support. Her strategic planning, leadership, and thorough understanding of the service system, including Medicaid rules and regulations, equip me to make a significant impact. Beth has over 30 years of experience in the Community Mental Health system and is a LMSW in both clinical and macro.

Cancellation Policy: If you do not cancel and do not attend, you will not receive a refund. Substitutions are permitted based on availability. Cancellations must be received in writing at least 14 days prior to the training for a full refund less the cancellation fee determined by the registration price below.

If cancellation is received less than 14 days prior to the training, no refund will be given. Exceptions must be approved by the Director of Education.
Special Accommodations: We strive to make all trainings accessible. Accommodation requests (e.g., mobility assistance, interpreters, accessible materials) can be submitted during registration or by email at least 14 days before the training. We will make every reasonable effort to fulfill requests; however, some accommodations may require additional time or depend on vendor availability. We will attempt to support on-site or late requests when possible but cannot guarantee availability.
National Accreditation Rules: National Accreditation rules indicate that if you are over five (5) minutes late or depart early, you forfeit your continuing education hours for the entire training. Please note that this is a National rule that CMHA must enforce or we could lose our provider status to provide continuing education hours in the future. This rule will be strictly followed.
Evaluation: Each participant is required to complete an evaluation of the course and the instructor. If you have any issues with the way in which this training event was conducted or other problems, you may note that on your evaluation of the training event or you may contact CMHA at 517-374-6848 or through our website for resolution.
Severe Weather Policy: Events will take place as scheduled, and we will not be able to refund registration fees. In the event of severe weather, please check the CMHA website for scheduling delays and event updates.
